Tiba2 ada jugak yang tanya....
"Betul ke awak nak jadi doktor ni?Tak ada life tau...saya pun rasa macam nak quit jer"
Mak aiiiih....sampai gitu sekali...Tapi itu adalah kata2 dari seorang HO (housemen officer) dan saya sedia maklum bahawa dunia HO ibarat 'a living hell'so no wonder la HO selalu cakap macam tu :P
Tak kiralah nak cakap apa pun.Saya dah penat cuti sampai tak tahu nak buat apa.Kerja dah,main masak2 pun dah (bolehla setakat nak makan family2 ni),jadi bibik pun dah,jalan2 makan angin pun dah.Jadi,sekarang,back to my real track lah kan.Kalau dulu,ye lah,nak sangat cuti lama2.Sekali bila dah dapat,ternganga.
Our first posting was Obstetric wards (include labour rooms).So every morning about 7.30am,we have to be in wards,followed MOs' or HOs' doing round before real ward round together with specialists and sincerely admit,I learnt a lot.A lot.If before,during clinical teaching 3rd year,sometimes I felt like a foolish,knew nothing and understood things for the sake of passing exam!Giving managements without priority,miserable history,stoppable physical examination bla2...But now,although not that much,at least I think I could digest a little bit what have been discussed by the teams (provided u already cover all the patients a night before).
For the example,how to manage a postdate patient?Doctors dont induce labour automatically but there are indications whether to induce labour or just wait for spontaneous normal delivery.If mom is free of medical illness and the fetus condition is good,so why we need to induce?Let say doctors need to induce labour,what if the induction failed?Then the option of C-sec comes as the priority.All this made me think 'owh ok I cannot jump to C-sec directly..there IS another option which I need to consider first'
After ward round,we moved down to labour rooms,observed any delivery regardless of vaginally or by Caeserean section in operation theater,learned anything that can be learned,CTG,partogram,blood taking,IV line set up,ultrasound,vaginal examination(VE),assisted doctors during delivery or played around with the babies (owh they are really2 super duper cute...rasa macam nak lambung2,golek2 jer babies tu semua).And I'm done with my VE!On the patient of course!Heeee...VE is a medical term and orang kita panggil 'seluk dalam' untuk tengok jalan dah bukak banyak atau belum.Thanks to Dr Rosmah for the chance given.Kalau tiada yang pertama,mana nak datang yang keduanya :D
Kalau tak ada case kat labour room,kami ada group discussion.Tak pon,cilinical teaching.By the way,teaching with Dr Ramli was super awesome.Sooo ke-bapak-an.Not forgotten,to doctors in team A and C and also who are in labour rooms,thanks a lot for acknowledge us.And nurses yang baik hati lagi budiman,toche2 :D
Bila malam,kami ke ward untuk cover patient dalam team masing2.Balik bilik,revise patients' problems and prepare for any question that might be asked on the next morning.Kadang2 kami turut sama 'oncall' kat labour room tapi sampai dalam pukul 11/12 malam jer lah.Tak adanya kami nak tunggu sampai pukul 6 pagi.Kalau sebelum ni berhadapan dengan complication of postdate pregnancy which was meconium aspiration (baby dah termakan najis sendiri dalam rahim sebab lambat sangat keluar),just now I managed to observe one more which was removal of cerclage (a cervical stitch which is used for the treatment of cervical incompetence).
And next week we are supposed to be in Gynaecology ward tapi saya tak nak!!Seriously I'm sooo into this field.Pegang2 perut ibu2,rasa baby tu gerak2 kat dalam,merasai sama keghairahan ibu tggu anak dia lahir (esp kalau anak sulung),berhadapan dengan ibu yang pucat lesi masa tgh teran,melihat senyuman dan air mata si ibu bila anak yang dikandung sekian lama kini dalam dakapan dan saksi bagaimana seorang suami tak lepas tangan daripada mengusap kepala isteri,kadang2 dibisikkan kata2 semangat.Yes!Air mata saya bergenang masa mula2 bayi keluar.A feeling when u saw a baby was delivered safely,cukup sifat dan sebagainya,it was undescribeable!
